The registration fee of $295 paid by March 10, 2002 ($345 after that date) includes conference handouts, breakfast on Thursday and Friday, refreshment breaks, lunch on Thursday, and Thursday evening Social and Vendor Demonstrations. The fee does not include hotel accommodations. Fees may be paid by check, money order, or purchase order. Please make all remittances payable, in U.S. Funds, to Arizona State University. Payments by VISA, MasterCard and American Express are also accepted.
Enroll by Fax, E-mail, Internet, Phone or Postal Mail
To reserve a space at the conference, fax the enrollment form as soon as possible. Be sure to note the method of payment. If you don't have access to a fax machine you may call (480) 965-1740 or E-mail asu.cpd@asu.edu. Mailing the enrollment form is only necessary if you will be enclosing a check. A confirmation letter will be faxed or mailed to you shortly after receipt of your enrollment.
